I understand that full payment is due upon receiving an invoice from the CSO-HNS, and that I will not be eligible for a payment schedule.

I understand that each sponsorship opportunity is subject to availability. While the CSO-HNS will update the list of available opportunities as they are reserved, I acknowledge that I will not be entitled to my selected sponsorship until it has been officially confirmed by the CSO-HNS office via email.

I have thoroughly reviewed the off-site sponsorship prospectus and fully understand the benefits to which I am entitled, as well as the limitations associated with my status as an off-site sponsor.
As an off-site sponsor, I acknowledge and understand that this sponsorship is an exclusive opportunity that does not provide the same benefits as the Exhibitor, Bronze, Silver, Gold, Platinum, or Diamond sponsorship levels.

I understand that my company’s attendance at the event is neither required nor compensated by the Canadian Society of Otolaryngology – Head & Neck Surgery (CSO-HNS). If I wish to attend, I am responsible for purchasing admission at my own cost, unless my sponsorship value exceeds $3,000.00 CAD. In that case, I will receive one complimentary admission ticket.

Please note that this complimentary admission excludes any travel and accommodation expenses, which I will be responsible for covering on my own.
