Please enter your first, second and third choice of booths. PLEASE NOTE: that the "VIP Sponsor Atrium" is reserved for Diamond, Platinum and then Gold sponsors first. Top tier sponsors may chose to be in the main "Exhibits Hall" as well. There are benefits to being in both spaces.

Booth Allocation: The Sponsor/Exhibitor understands that space will be assigned on a first come, first served basis, with possible limitations on the number of spaces per company. Payment Terms: Payment shall be made by credit card or electronic transfer to CSOHNS prior to the commencement of the conference. Full Payment is due within 14 days of receiving the invoice. Cancellation Policy: Cancellations must be submitted to the Organizer in writing, regardless of the timing of the cancellation. Cancellation must be directed in writing to exhibits@csohns.org. The date of receipt of the Sponsor/Exhibitor’s written notice of cancellation will be the official cancellation date. If exhibit space/sponsorship is cancelled on or before March 1, 2026, the Sponsor/Exhibitor will be responsible for 50% of the total fee. No refund will be provided for cancellation requests made after March 1, 2026. All funds paid are non-transferable. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the Province of Alberta. Acceptance: The Sponsor/Exhibitor agrees to all the rules, provisions and stipulations set out in the information package and further agree that the Sponsor/Exhibitor shall assume responsibility for and liability for losses, damages and claims arising out of injury to persons or damage to displays, equipment, or other property brought onto the premises of the Calgary Telus Convention Centre and agree to indemnify, defend and hold harmless the Canadian Society of Otolaryngology - Head & Neck Surgery, the Calgary Telus Convention Centre and its owners, agents, and employees against all claims or expenses for such losses, including reasonable legal fees, arising out of the use of the conference premises excluding any liability caused by the negligence of the Canadian Society of Otolaryngology-Head & Neck Surgery or the Calgary Telus Convention Centre or its owners, agents and employees. The Sponsor/Exhibitor understands that neither the Canadian Society of Otolaryngology-Head & Neck Surgery nor the Calgary Telus Convention Centre maintains insurance covering the Sponsor/Exhibitor 's property or loss of revenue and it is the sole responsibility of the Sponsor/Exhibitor to obtain such insurance.
